Welcome back—third round. I pulled listener questions from our last two episodes, so let’s run a tight Q&A. First up: beyond wrinkles and sun spots, what really drives that harsher, less balanced look with age, and how do you head it off?

Skin collagen loss is only one layer. Ligaments loosen, facial fat pads deflate—especially midface—and bone resorbs at the orbit, maxilla, and jaw, so the face loses support and harmony. Prevention has to be holistic: protect bone and muscle with training and diet, avoid big weight swings, manage sleep, insulin resistance, and inflammation, fix bruxism and gum issues, and use targeted aesthetics to preserve proportions—not chase every line.

UV damage stacks up over decades, and tension from grinding and screen stress doesn’t help.

Start with health basics, then treat folds and shadows. Facial training and gentle massage can relax overactive depressors like the platysma and support lifters such as the zygomatics; consistency matters. We’re all a bit asymmetric, so be conscious of one‑sided chewing or tighter muscles, and in stubborn cases a touch of botulinum toxin can balance tone—while keeping sun exposure sensible year‑round.

GH falls after youth, but cartilage in the nose and ears stays responsive, so it can thicken slowly, and other tissues shrinking makes them look larger. High‑dose GH abuse exaggerates this; for medical use, tiny doses or GHRH‑pathway peptides like ipamorelin or CJC can be safer when carefully titrated. Testosterone and estrogens influence skin oil and texture, but keeping youthful physiologic ranges is the goal—not supra‑physiologic anti‑aging hacks.

What’s the minimum effective at‑home routine—the few things that actually move the needle?

Three, maybe four. Daily broad‑spectrum SPF 50 plus is the biggest win. At night, a retinoid—tretinoin is best studied—introduced slowly to build tolerance before stepping up strength. Then barrier support: a gentle low‑pH, soap‑free cleanser and a moisturizer with ceramides, niacinamide, or panthenol; many sunscreens include these to simplify. Optionally add an antioxidant like vitamin C or fat‑soluble astaxanthin; peptide serums are interesting but less proven.

UV is the main driver of extrinsic aging, so protection leads.

Your skin has to last a lifetime. Favor morning or late‑day light, wear hats and UPF clothing, seek shade, and don’t rely only on lotion—especially around midday.

Best evidence‑based sunscreen filters right now?

For mineral, non‑nano zinc oxide covers UVA and UVB well; I avoid titanium dioxide, especially nano forms, for weak UVA coverage, skin penetration, and environmental concerns. Among modern organic filters, look for bemotrizinol and bisoctrizole, Uvinul A Plus and T 150, ethylhexyl triazone, and Mexoryl—photostable with low absorption. I steer clear of oxybenzone, avobenzone, and octocrylene due to irritation and endocrine issues. These newer filters are common in Europe and Asia and should expand elsewhere.

Daily sunscreen or follow the UV index?

Make SPF a daily habit; the index only predicts how fast you’ll burn and misses cumulative and window exposure. When the index hits around three or higher, add hats, sleeves, shade, and keep midday stints short.

Are people overdoing skincare?

All the time. Cut the lineup to three or four evidence‑backed steps and prioritize reapplying sunscreen, even with makeup. Most trendy serums repeat the same function and just get in the way.

Thoughts on castor oil, beef tallow, and egg yolks?

Castor oil softens but lacks strong data for growth or anti‑aging. Beef tallow is purely occlusive—it can clog, turn rancid, and flare acne, rosacea, or seb derm; save it for very dry body skin if you must. Egg yolk has compatible lipids and amino acids but can trigger allergies, so avoid on irritated skin. None replace retinoids or SPF, and natural oils offer minimal sun protection.

If money were no object, what whole‑body rejuvenation would you actually do?

Big‑budget doesn’t replace judgment. I’d scale diagnostics first: monthly broad labs, frequent epigenetic clocks and telomeres, VO2 max and body comp, plus ultrasounds every six months and whole‑body MRI every couple of years. Therapies I do now—cautious peptide use, IV senolytics with quercetin and dasatinib, and IV or subQ NAD—are layered on top of lifestyle. IV exosomes lowered my epigenetic age by roughly four and a half years in my n‑of‑one, with close monitoring. I’d consider therapeutic plasma exchange to lower inflammatory cargo, MUSE stem cells for safer, multi‑tissue support, and, when viable, gene therapies like klotho for longevity and APOA1 Milano for cardiovascular protection. None of this replaces blood pressure, glucose, lipids, fitness, and sleep—the real foundation.

There’s no proven human therapy that reverses aging outright, so we’re optimizing healthspan, slowing the slope, and cutting disease risk.

Exactly—we’re using the best surrogates we have while better markers and trials catch up. On that note, which biomarkers matter most? The unsexy ones lead: blood pressure, lipids and glucose control, kidney and liver function with albumin, C‑reactive protein, body composition, VO2 max, and grip strength. Second tier includes heart rate variability, gait speed, muscle mass, balance, and bone density. I add PhenoAge, GrimAge, and DunedinPACE for context—GrimAge is steadier, PACE jumps around with recent behavior—plus telomeres and glycation clocks for extra perspective.
