Absolutely. I start at the genetics-environment interface: intergenerational trauma and cortisol signaling, plus inherited conditions such as endometriosis, fibroids, and PCOS.
Let’s go decade by decade. Once puberty begins, what should women track?
For teenagers, cortisol is especially interesting; estrogen and progesterone vary while the system matures. Regular cycles can provide a reference point, though the twenties and thirties are more useful. In your twenties, get a baseline for estrogen, progesterone, testosterone, DHEA, estrogen metabolites, and the microbiome. For a typical twenty-eight-day cycle, I’d often test around day twenty-one or twenty-two: dried urine shows metabolites, while blood is the cheaper, insurance-friendly snapshot.
I also like nutritional testing: antioxidants, B vitamins, glutathione, minerals, especially magnesium. Measure red-blood-cell or whole-blood magnesium. Lower perceived stress with an à la carte menu—meditation, yoga, breathwork, sex, orgasm, connection, feeling heard and loved. High adolescent androgens can foreshadow PCOS, a major cardiometabolic issue later. I’m a huge fan of continuous glucose monitors for changing behavior, though insulin can shift before glucose.
Don’t overlook sleep, alcohol, high perceived stress, poor-fit food, toxic relationships, isolation, or exercise that doesn’t suit your body. Chronic running drove my cortisol higher; Pilates and yoga served me better. Oral contraceptives offer reproductive freedom and lower ovarian-cancer risk, but I’m concerned about synthetic progestins and loss of free testosterone for sensitive women. By your thirties, know your hormonal phenotype before perimenopause. Consider a CAC score by forty-five, earlier with premature heart disease or risks such as PCOS.
