Fasting restores the missing physiology: after approximately twelve hours you burn glycogen, then you start pulling fat—first from the visceral stores.

Prevention of heart disease, the number one global killer. The heart can fail as a muscle, its valves can calcify, arteries can block, and even the tiny capillaries can misfire. I have been operating for about thirty-five years, treated in excess of thirty thousand cases. Heart attacks are not a slow squeeze; they happen when a small plaque ruptures, the blood sees the crack, and a clot suddenly seals the artery. The driver is inflammation, plus today’s perfect storm: too much plaque formation, unstable plaques, and more clotty blood.
Walk me through the emergency fix and why a twenty-eight-year-old had a heart attack.
We thread a catheter from wrist or groin, wire across the blockage, balloon it, then deploy a stent. That twenty-eight-year-old was overweight with a protruding belly, fatty liver, and prediabetes—normal A1C but sky-high insulin. Insulin is atherogenic: it thickens vessel muscle, constricts arteries, promotes clotting, and fuels inflammation. When I began testing insulin, not just glucose, everything clicked—many so-called non-diabetics were insulin resistant.
Explain glucose and insulin simply—and why frequency of eating matters.
Glucose glycates proteins, enzymes, and vessel walls—it ages and damages them. Insulin shuttles glucose into cells, liver, and muscle. But if you snack on sugars and fast carbs every few hours, insulin stays elevated for hours, the body becomes resistant, and you need even more insulin for the same effect. High background insulin builds fatty liver and preferentially packs fat around organs—the visceral fat that inflames everything. Calorie cutting is not the same as fasting: calorie restriction slows metabolism and sheds muscle; fasting drops insulin, mobilizes fat, and targets visceral fat first.
What fasting actually works—and does it differ for women?
Start with twelve-twelve for two to three weeks, then eighteen-six with only water, black tea or coffee between. For major weight or diabetes reversal, I use one forty-eight-hour fast weekly or a three-day water fast every nine days with one meal a day between. Most women tolerate this well; exceptions are those pregnant or trying to conceive. I have supervised extended medical fasts with dramatic improvements: normalized blood pressure, reversal of diabetes, deep visceral fat loss, and better skin recoil—fasting physiology is different. After approximately twelve hours, fat releases, the liver makes ketones—cleaner fuel that boosts brain-derived neurotrophic factor, mobilizes stem and endothelial progenitor cells to repair vessels, and raises growth hormone. Train near the end of a fast; prioritize resistance and high-intensity intervals. Women may struggle with long steady-state cardio fasted, but do very well with resistance and HIIT.
Best training for heart health?
Too much chronic cardio increases inflammation and coronary disease in my clinic. Do fifteen to twenty minutes of aerobic work for endurance, then focus on resistance and HIIT: thirty to forty-five seconds hard, equal rest. The rest lets your antioxidant systems clear the metabolic mess and keeps physiology cleaner.
Do you still see tragedy despite prevention—and can a ‘healthy’ person have heavy calcification?
I cannot stop death, but I aim to extend healthspan. I have seen fit, non-smoking, normotensive patients with very high coronary calcium. Dig deeper and you find hidden inflammation—often the gut. The microbiome makes micronutrients and maintains the gut barrier; when it leaks, bacterial wall fragments flood the liver, drive fatty liver and systemic inflammation. Processed foods and sugar starve good bugs; restoring fiber, fermented foods, and nutrients flattens calcium progression. Aim for a coronary calcium score of zero. I stop calcium supplements; the real issue is low vitamin D3 and K2, especially in those on warfarin, which depletes K. I supplement K2 because modern diets lack fermented foods. Most people are fiber deficient; I use inulin with FOS and encourage diverse vegetables and spices. Sleep seven hours—one bad night makes you insulin resistant the next day.
Toxins and mold—how do they fit into heart risk? I even found mold in a steam room at home.
Pesticides, herbicides, plastics, PFAS, BPA, heavy metals—and mold—disrupt metabolism and provoke inflammation. Many homes, possibly around seventy percent, have water-damage mold. It can colonize sinuses or gut and re-seed you, keeping the innate immune system switched on. When we find and treat mold and support gut and liver detox, symptoms improve and coronary disease progression slows.
Diet traps people think are healthy?
Processed foods in boxes; most breads except occasional fermented sourdough; white rice unless you soak to reduce arsenic, cook in plenty of water, discard, cool, and reheat to form resistant starch; charred or over-fried foods that create advanced glycation end products; excess fruit due to fructose and fatty liver; and vegetable seed oils with high omega-6. Use extra-virgin olive oil for salads; for heat, small amounts of butter, ghee, or coconut oil.
What signs can I spot, and what about palpitations, caffeine, and the vagus nerve? Also, I got weeks of palpitations after a one-hundred-kilometer bike push.
A protruding belly is the red flag—visceral fat. Joint pains often reflect systemic inflammation and improve when we fix gut, sleep, stress, and eating patterns. Oral health matters: poor dental hygiene and chronic sinusitis, especially fungal, link to valve calcification and coronary disease. Caffeine in moderation is fine, but excess mimics adrenaline and dehydrates; palpitations in otherwise healthy people are often autonomic imbalance—too much sympathetic, too little parasympathetic—frequently gut-driven. Your over-exertion likely shunted blood from the gut, briefly injuring the enteric nervous system and blunting the vagus, provoking arrhythmias. The vagus nerve is the body’s repair channel—optimize it by fixing the gut, ensuring omega-3 DHA, and daily parasympathetic ‘hacks’: four-in eight-out breathing for ten minutes, eye movements, cold water to eyes or front of neck, humming or singing, laughing, gentle Valsalva. A calmer vagus improves heart rate variability, lowers blood pressure, inflammation, and clot tendency. Now, LDL—‘bad cholesterol’—matters in context, which we can unpack next.
LDL itself is not the villain; small, dense, oxidized LDL is. Those particles damage the glycocalyx and endothelium, macrophages swallow them, become foam cells, and build plaque. I order LDL sub-fractionation: large, fluffy LDL is far less concerning; small, dense points to inflammation driven by sugar, excess omega-6, advanced glycation end products, toxins, and lipopolysaccharides from leaky gut.
How do I lower LDL naturally—are eggs okay—and what about statins long-term?
Vegetarian eating can drop LDL, but the real goal is reducing small, dense LDL by removing those five drivers. Eggs are no concern. Statins lower LDL, but not all are well tolerated—twenty to thirty percent get muscle loss, metabolic or cognitive issues; monitor liver—and lowering LDL does not automatically mean fewer events unless you are targeting oxidized, small, dense particles.
Is high blood pressure just narrowed pipes?
High insulin blunts vascular dilation, so pressure rises; when I lower insulin with fasting and diet, blood pressure often normalizes. Sleep apnea and hyperinsulinemia are common causes—“essential” hypertension is not destiny.
What should we eat for breakfast for heart health?
For most adults, skip breakfast and aim for an eighteen-hour daily fast. If you must eat, have kefir with a few berries and inulin. I do one meal a day several times a week and feel more energy.
What does your own diet and supplementation look like?
Small amounts of organic, grass-finished meat with vegetables; Indian-style lentils and curries; minimal chapatis, naan rarely. Supplements: vitamin D3, K2, omega-3 fish oil, vitamin C one gram, inulin, kefir, magnesium, nattokinase about eight thousand, and a spore-based Bacillus probiotic that survives stomach acid and germinates in the gut.
What is the most important thing people should do now to assess risk?
Get a coronary calcium score—zero is reassuring; any calcium means atherosclerosis and you should start prevention. Order an inflammation panel like Cleveland Heart Labs: LDL particle size and oxidation, CRP, interleukin six, tumor necrosis factor, A1C, and more. Mental fog, poor sleep, and depression often reflect inflammation; an anti-inflammatory diet frequently lifts mood.
Our closing tradition: the hardest day of your life, and how did you overcome it?
My father’s passing. I second-guessed myself, but it taught me humility and presence. Lifestyle changes gave him roughly three extra decades to eighty-nine; he ultimately died after a gut toxin overwhelmed him. The enduring lesson: live and work fully in the present moment.
